Description

A mental health and wellbeing social prescribing service run by the charity Pentreath, with Health and Wellbeing Coaches based at Carnon Downs Surgery and Chacewater Health Centre. Coaches support patients to set goals, build confidence and connect with community groups and self-management support. Referrals are made via the GP surgery.
