Recently, our Home and Hospital Service helped Elizabeth return home after a lengthy hospital stay. This short case study highlights some of the support our Home and Hospital Service can provide for patients in Cornwall.

Returning Home
Elizabeth was initially hesitant to be discharged - but following a conversation and assessment with our discharge coordinator, we could alleviate Elizabeth’s concerns and find the right support to meet her needs. Elizabeth’s primary concern was the condition of her home, especially after such a long time in hospital.
With the support of the hospital team, we arranged for her home to be cleaned before her return. We also organised for meals to be delivered for the next couple of days - ensuring Elizabeth had warm, healthy food once she left the hospital.
After further discussions with Elizabeth, we were able to ensure she was able to access a strong support network - Elizabeth now attends the Age UK Cornwall community hub two days a week and is connected to someone she used to go to school with - providing companionship and friendship.
The Falmouth community hub also provides a wide range of activities in a safe and friendly environment. These activities help build resilience and increase independence. Elizabeth is attending a mix of activities and hopes to join more groups in the future.

Caring at Home
Elizabeth’s daughter has also been struggling to cope, and as her main carer, she felt that all of her mum’s care would be left to her. This responsibility was overwhelming and only increased Elizabeth’s anxiety.
However, with support from the Carers Service (Cornwall), we have helped her daughter access a carers
grant, a network that she can access and provided her with essential respite. Knowing that Elizabeth is cared for within our hub has meant that both Elizabeth and her Daughter can enjoy free time - concentrating on what is important to them.
