Support people with Type 2 diabetes

Diabetes and You is primarily intended for adults newly diagnosed with Type 2 diabetes. The programme may also be appropriate for people living with Type 2 diabetes who would benefit from a refresher or updated education to support self-management.

General Practices and Primary Care Networks are encouraged to offer Diabetes and You to eligible patients and, wherever possible, provide access through locally delivered courses delivered by trained Diabetes and You educators.
