Who we are

Home Group is committed to reducing the impact of homelessness and social disadvantage through its provision of housing related support to meet a wide range of needs. Home Group is committed to providing its service within a framework of equal opportunities, anti-discrimination and respect.

Empowering Independence Service - West (EI West) is a service providing accommodation and outreach support in the community for customers with complex support needs in Cornwall. The work undertaken is funded through Cornwall County Council’s Adult Social Care (ASC) “Social Inclusion and Empowering Independence” contracts from October 2019.

Our aims and objectives

Our key aim and objective is providing short term Supported Accommodation at Rosehill Meadows (typically up to two years) as well as Community Outreach support (typically up to six months) with independent living skills and self-management of health and wellbeing. This will include but is not limited to supporting people with maintaining accommodation, managing domestic tasks, managing finances and budgeting, accessing employment, education and training, and attending health appointments. We also provide a drop in on Monday at Hayle library 3-5 till 21^st October when it will become 1:30-3:30 and Wednesday at Penzance St John’s Hall 9:30-11:30.

Rosehill Meadows provides supported Accommodation – for people requiring low to medium levels of support, and limited tolerance to alcohol/drugs.

Who is the service for?                                                                                                                                                                                                                    

People accessing the services will have support needs related to one or more of the following:

·       Mental health/emotional wellbeing

·       Alcohol/drugs

·       Acquired brain injury

·       High risk behaviours, such as hoarding and self-neglect

People accessing the services may also have experienced other multiple disadvantages related to rough sleeping, homelessness, contact with the criminal justice system, social isolation, poverty and/or abuse.

Referrals and Applications

We set eligibility requirements based on a number of factors, including the aims of the service, the people we provide the service for and any conditions agreed with the Council who commissioned the service. To be eligible for the residential part of the service, the customer must be:

• A Single person
• Aged 18 or over
• Not be considered a threat to others
• Be able to self-medicate prescribed medication, or have alternative input to support them with this
• Be able to comply with the responsibilities of maintaining a tenancy or licence agreement and house rules with appropriate support.
• Be able to self-cater and keep their flat to an acceptable standard, with support if required.
• Engage with staff to formulate their support plan and attend weekly linkwork sessions
• Engage with other organisations providing necessary support as required.
• Be addressing issues relating to the use of alcohol, drugs or substances if appropriate
• Be expected to be able to move on to suitable independent accommodation with support
• Have no pets (exceptions may be made e.g. for a small caged pet but must be agreed in advance)

The above factors will be taken into consideration at a formal induction / assessment before the applicant is offered a place at the service. Priority will be given on the basis of need and urgency of support and accommodation.

The services will be direct access meaning that anyone will be able to make a referral. The services will focus on preventing, reducing and delaying individual’s needs for care and support in accordance with the Care Act (2014). People will not need to be eligible for social care to access the services.
