Social Prescriber Link Worker

Hi. I am a Social Prescriber Link Worker based in the Liskeard area of Cornwall. I am usually based in the three GP surgeries in that area and my role is to make links for patients.

Sometimes patients may need a little help that health practitioners can’t support them with and that there is no medicine for. This may include increasing their social circle by joining in groups or activities with others - art / crafts / education / training, or maybe increasing physical activity – bowling / walking / volunteering opportunities, or they may need help reducing barriers they face that are preventing them from doing what they used to do or from trying something new, maybe they need help seeking advice with housing / benefits / debts / improving thir chances of finding employment or signposting to help with mental health.

Patients are referred by their GP or a healthcare professional. Once referred, I will contact the patient and invite them to come in to discuss their needs and then link them to opportunities within the local community.
