HOSPITAL DISCHARGE SERVICE
We aim to reduce loneliness and social isolation to improve health and wellbeing for all adults living in Cornwall. Our Hospital Discharge service provides further help to support an individuals recovery following a stay in hospital, or before, or upon discharge.

We provide social and practical support following a hospital stay, working together to reduce loneliness and social isolation.
What's available:
- Social inclusion plan
- Telephone wellbeing calls for up to 6 weeks
- Information to keep people warm
- Information on community transport
- Signposting to services and activities
- Free benefits check and access to current grants
Our Hospital Discharge Officer will co-produce with you an action plan. In some cases a Wellbeing volunteer can be arranged. All our staff and volunteers have full DBS certificates.
We are commissioned by the NHS and Cornwall Council, as such this service is fully funded - free to people who access the service.
