INCLUSION MATTERS
We aim to reduce loneliness and social isolation to improve health and wellbeing for all adults living in Cornwall. Our Hospital Discharge service provides further help to support an individuals recovery following a stay in hospital, or before, or upon discharge.

Are you lonely or isolated?
- Is this affecting your recovery, health and wellbeing?
We can help:
- Provide telephone welfare calls for up to 6 weeks.
- Identify safety concerns in the home.
- Provide information to keep people warm.
- Support with transport options.
- Arrange for someone to call round for a chat.
- Signpost to community services and activities.
- Signpost to information and advice on finances, including welfare entitlements and grants.
Our Hospital Discharge Officer will co-produce with you an action plan. In some cases a Wellbeing volunteer can be arranged. All our staff and volunteers have full DBS certificates.
We are commissioned by the NHS and Cornwall Council, as such this service is fully funded - at no cost.
