Overview
Inclusion Matters project is a partnership service that aims to make it easier for residents of Cornwall to find information and advice they need to increase their wellbeing, stay healthy, get active and improve their connection to their community. Focusing on social inclusion and empowering independence.
- Help people identify local support services, networks and activities, and to make links in their local community.
- Link people to volunteers that offer wellbeing and practical support, to people in their own homes, and following hospital discharge.
- Help people identify volunteering opportunities within their local communities or within projects.
- Provide information, advice, support and guidance to community individuals, veterans, unpaid carers of all ages, groups and organisations.
- Provide help and support for community groups, to set up, source funding, training and connections.
- Bank of trained volunteers & volunteering opportunities available, to reduce loneliness and isolation, befriending or providing practical help.

Hospital Discharge
Our Hospital Discharge Team can support you with an action plan upon discharge or just after for when you are back home.
We can help with the following:
- Identify safety concerns in your home
- Provide information to keep your home warm
- Collect your shopping or prescriptions
- Support with transport options
- Signpost to community services and activities
- Signpost to information and advice on finances, including welfare entitlements and grants.
- Arranging for someone to call round for a chat, or over the telephone.
