Social prescribing

Social prescribing is when health professionals refer patients to support in the community, in order to improve their health and wellbeing, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. Self-referral is also encouraged.

Social prescribing works for a wide range of people, including those:

  • with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing.

Supporting people to get online and use digital health resources can be crucial for: physical and mental wellbeing,  prevention,  self care, shared care and shared decision making, long term condition management appropriate use of urgent and emergency care.
